Homemaker Service offers a broad array of in-home services to meet the needs of consumers in their homes.
Attendant Care
Care for Adults
A program designed to offer individuals
with physical disabilities the ability to live independently in the privacy
of their own homes. The program assists consumers to remain at home, attend school, go to
work and participate in activities in the community.
Eligibility: Age 18 to 59, mentally alert with a physical disability that is expected to last for one (1) year or longer.
Personal Assistance Services
Care for seniors
A personal care services program designed
for the elderly. It is a consumer model of care offered through the Philadelphia Corporation For Aging. This program allows elderly consumers to hire and employ the caregiver they choose and trust to assist with the activites of daily living.
Eligibility: Age sixty (60) or above and a recipient of services through the Philadelphia Corporation For Aging and County of Delaware, Office of Services for Aging.
Financial Management Services
Hire Your own, we do the paperwork
Fiscal assistance is available to consumers receiving in-home services through Attendant Care, Personal Assistance Services (PAS) and other Medicaid Waivers. Consumers who choose to hire their own attendants become the employer of their attendant(s).
Eligibility: All consumers using the consumer model for Attendant Care, PAS and other Medicaid Waivers.
Skilled Nursing Services

Skilled Services available to patients are Skilled Nursing, Occupational Therapy, Physical Therapy, Speech Therapy, Medical Social Work Services, Home Health Aide Services.
Eligibility: Any individual age 18 and older who is homebound and needs temporary assistance requiring skilled home health services. A physician’s order is required.
OBRA, Independence and
CommCare Waivers
Homemaker Service offers assistance with daily living services to consumers receiving in-home service through these waivers. Please call our office for information about each of these waivers.
Eligibility: Age 18 and older is physically disabled with a disability that substantially impacts functional abilities.
Nursing Home Transition
Return home
NHT is a transition program designed to help nursing home residents who want to return home to live independently in the community and are able to do so with the assistance of home care services.
Eligibility: Age 18 to 59, resides in a nursing home located in Montgomery County, PA, desires to return to the community to live independently.
