Nursing Home Transition

Nursing Home Transition (NHT)

NHT is a transition program designed to help nursing home residents who want to return home to live independently in the community and are able to do so with the assistance of home care services.

Services Available

  • Individual evaluation and support from our staff.
  • Assistance with finding affordable housing if the resident needs help with housing
  • Assist with payment of the security deposit
  • Help plan the move including setting up the residence, purchase of furniture, lamps, linens, etc.
  • Arrange for adaptive equipment if necessary
  • Arrange for attendant care services or other in-home supports to assist in their transition to independent living

Eligibility

  • Age 18 to 59
  • Resides in a nursing home located in Montgomery County, PA, and desires to return to the community to live independently.

Cost to Patient

There is no cost for this program for those who qualify.
