Skilled Nursing Services

Authorized by a doctor for skilled care on a short-term basis.

The Skilled Care Division offers skilled nursing, physical therapy, occupational therapy, speech pathology, medical social services and home health aide services. These services are provided as ordered by the patient’s physician in their residence. The specific care services rendered are set forth in a plan of care developed by the patient’s physician, in collaboration with the skilled division professional staff.

Skilled Nurses

Skilled Nurses provide skilled nursing to patients in their own homes.

Physical Therapy

Physical Therapists provide evaluations and therapeutic services for persons with physical impairments and deficits in daily living activities.

Occupational Therapy

Occupational Therapists provide rehabilitative, vocational and educational activities to homebound patients with physical, developmental, emotional and social deficits.

Speech Therapy

Speech Therapists provide evaluative and therapeutic services for persons with language and/or speech disorders, which impair communication and/or swallowing.

Medical Social Work Services

Medical Social Workers provide services to resolve actual and potential social or emotional problems that are expected to be an impediment to the effective treatment of the patient’s medical condition and his or her rate of recovery.

Home Health Aide Services

Home Health Aides assist patients with personal care needs.

Eligibility Requirements

  • Any individual who is deemed homebound and needs temporary assistance requiring skilled home health services.
  • The patient is 18 or older.
  • The applicant must reside in Philadelphia County.

Cost to Patient

Skilled home care services are covered by Medicare, Medicaid and by many private health insurances. Call us and we can help determine your insurance coverage.

How to Enroll

A physician’s order is required for skilled home health care. A referral for skilled services can be made to Homemaker Service by having a physician or physician’s representative call 215-592-0008 and make referral for skilled care services.
